Required Student Health Forms

Our staff at Health Services is committed to maintaining a safe and healthy campus for our entire community. Below you will find a detailed list that explains your requirements to ensure compliancy with New York State Public Health Laws 2165 and 2167. These laws apply to all students taking six (6) or more credit hours.

Niagara University strictly enforces compliance with these laws. Students who do not submit the required information within thirty days of their first class will have their registration withdrawn. Withdrawn students cannot attend class, reside in college housing, access college technology, participate in or attend campus events. All requirements are due 30 days prior to the start of your classes.

Required Medical Forms - Undergraduate Students

Log into Medicat (use your purplepass sign on)

You must be logged in to perform the following activities:

  1. Click on Forms Tab
    • Click on NU Health History on-line portal form and complete
    • Click on TB Risk on-line portal form and complete
    • Click on Meningitis Declination on-line portal form and complete if you have not had a Meningitis Vaccination within the past 5 years.
    • Click and print applicable physical form (ie: Undergrad, International) Take to your physician and have your physician fill out, sign and date.
    • Go to the upload tab, choose “college physical” form, scan and upload.
  2. Click on the Immunization tab
    • Enter in dates of Measles, Mumps and Rubella vaccinations
    • Enter in dates of Meningitis Vaccine if it is within the past 5 years.
    • If additional vaccines and dates are required (ie: nursing) enter them here as well.
    • Go to the upload tab. Choose “immunization record” to upload. Scan and upload immunization record from your doctor.
    • International Students will need to submit their health insurance cards
  3. Click on Insurance tab
    • Click on the "add new" button
    • Enter in your insurance information
    • Go to the upload tab, choose “insurance card” and upload the FRONT AND BACK of your insurance card

Important - Please Review

For your convenience, Health Services does offer physical examinations and the MMR immunizations from September through May. To schedule an appointment for any of these services please call the Student Health Center at 716.286.8390.

The above information is due to the Student Health Center 30 days prior to the beginning of the semester.

Students who do not submit the required information will be excluded from class, the residence halls and prohibited from further registration at Niagara University, per university policy. Feel free to contact the Student Health Center at 716.286.8390 should you require assistance.
