Welcome from the Director
Dear Incoming Graduate Student,
Welcome to Niagara University! All graduate students are required to submit a completed physical examination (within one year prior to entering classes) and a medical history form prior to class attendance. Physical examination and medical history forms can be downloaded from this web site.
New York State requires students attending colleges and universities in New York to comply with Public Health Laws 2165 and 2167 for measles, mumps, rubella and meningitis, respectively.
Niagara University is committed to maintaining a safe and healthy campus community. Therefore, the university strictly enforces compliance with these mandates. Students must submit proper proof of immunity to the Student Health Center prior to the first day of the semester. Students who do not submit the required information will be excluded from class, the residence halls and prohibited from further registration at Niagara. Proof of immunity as defined by New York State can be found on this web site at "Health and Immunization Requirements.”
The Student Health Center does offer physical examinations and immunizations needed for class attendance. These services are provided from September until May. To schedule an appointment for these services please call the Student Health Center at 286-8390. Students will be charged a fee for these services. Payment is required at the time of service. Physical examinations cost $75 in the Health Center.

Physical examinations and immunizations can also be obtained through county health departments. If you are an international student please read on this web site "International Health Insurance Forms."
